Description

Health coaching is a free phone support program to help individuals manage their conditions and take control of their health.
Individuals living with chronic health conditions spend very little time with health professionals. Most of the time they are managing on their own.
Health coaches provide support and guidance to support people to make the healthy changes they want to work on such as managing their health condition(s) and its complications, finding helpful community resources and taking action to live a healthy life. Their expertise stems from their shared experience with chronic conditions.

The Health Coaching program is funded by Ontario Health East. Please visit www.livinghealthychamplain.ca to learn more about the work we do.
